APPROCHE DE LA RÉÉDUCATION
APRÈS RÉPARATION ARTHROSCOPIQUE DE LA COIFFE
- Les principes
. limiter les contraintes lors des phases inflammatoires et prolifératives et éliminer tout exercice d’étirement en particulier dans les amplitudes extrêmes pour ne pas entraîner de stress mécanique sur la réparation et limiter une inutile augmentation de la réponse inflammatoire
. augmentation progressive des contraintes
- modalités pratiques
- tout protocole de rééducation débute par une communication etroite entre le chirurgien et le kinésithérapeute (transmission d’un compte rendu opératoire où toutes les données chirurgicales importantes sont listées : quels tendons, quelle taille de rupture, quelle qualité tissulaire, quelle type de réparation et quels gestes associés ?
- cette communication etroite doit se poursuivre durant toute la rééducation
- après analyse de la rupture et de la réparation effectuée, le patient se voit proposer un protocole adapté
- les protocoles de la première période
- protocole protecteur : ruptures transfixiante stade 2 avec tendon de qualité médiocre (fragile, perte de substance), larges, rupture massives, reinsertions itératives. Immobilisation 6 semaines, pendulaires pour les ruptures larges, immobilisation stricte pour les autres
- protocole spéficiques petites lésions : rupture partielle en particulier profonde ou de taille limitée, ruptures à risque de raideur (calcifications, capsulite preopératoire, réinserion labrale associée). Immobilisation 4 semaines.
- protocole standard : rupture transfixiante 1 ou 2 tendons : stade 1, stade 2 avec tendon de qualité satisfaisante
- Pour les protocoles petites ruptures & standard, le travail des rotations sera précisé :
- rupture subscapularis > 30% du tendon, rupture emportant la corde antérieure du supra- supinatus re1 passive limitée à 0° vs 30°
- rupture subscapularis > 30% du tendon, rupture emportant la corde antérieure du supra- supinatus travail différé de la rotation interne au
- modalités pratiques seconde période (S+6 S+12)
- période de vulnérabilité de la réparation
- sevrage progressif de l’attelle
- travail passif pour récupération des amplitudes sans travail excessif de la ri.
- travail actif aidé (sec ou immersion)
- renforcement musculaire proscrit
- attention aux patients dont la récupération est « facile »
